Non-supplement papers

Author’s Statement and Copyright Release form – all authors must complete and sign before paper is published.

Change of Authorship form – submitted if change is made in authorship.

Author Reprint Order form – provided to author by Dartmouth Journal Services when author proofs are sent.

Free Access Choice form – submitted if author has Wellcome Trust, MRC, or HHMI funding OR if the author chooses to make paper free online immediately upon publications (associated fee).

Supplements

Conflict of Interest Disclosure form – Supplement Coordinators must complete and return.

Supplement Data form – Supplement coordinators will receive from ASN office once supplement proposal has been approved. Organizer must complete and return.
